Osteopathic Manipulative Treatment for Refractory Gastroesophageal Reflux Disease (GERD)


Abstract

Introduction/Background: Gastroesophageal Reflux Disease (GERD) is a common problem that is found in a variety of demographics and can significantly affect a person’s quality of life. Lifestyle modifications and Proton Pump Inhibitors (PPIs) are standard treatments for these patients.

Case: A 45-year-old female with a past medical history significant for a cerebral vascular attack in June of 2020 was referred to the OMT clinic by her primary care physician for R shoulder pain. Review of systems was positive for severe heartburn, causing her to vomit multiple times daily requiring her to carry an emesis bag. She took 4 tablets of Famotidine daily with minimal relief. Physical exam revealed direct tenderness to her epigastric area. OMT was directed towards the following pertinent somatic dysfunctions: celiac ganglion restriction, cervical and suboccipital muscle hypertonicity, mesenteric restriction, and multiple cranial strain patterns. Numerous treatment modalities were used including myofascial release, OA decompression, cervical soft tissue techniques, and osteopathy in the cranial field.

Results:  Over 8 OMT visits the patient experienced significant improvement in her GERD symptoms, citing decreased frequency and severity of heartburn. Her Famotidine intake decreased to PRN, and vomiting became rare. As per her original complaint, her shoulder pain improved, with range of motion increased >50% in abduction and flexion. She continues to receive regular OMT for these and other complaints.

Discussion: This case report suggests OMT may provide benefit in the treatment of refractory GERD, improving the quality of life in these patients. Limitations include that this is a single case report for which the patient received treatment for a condition that was not her original chief complaint. Further research is warranted to explore this potentially beneficial area of treatment.
